  • MedHELM: A Comprehensive Healthcare Benchmark to Evaluate Language Models on Real-World Clinical Tasks Using Real Electronic Health Records

    Large Language Models (LLMs) are widely used in medicine, facilitating diagnostic decision-making, patient sorting, clinical reporting, and medical research workflows. Though they are exceedingly good in controlled medical testing, such as the United States Medical Licensing Examination (USMLE), their utility for real-world uses is still not well-tested. Most existing evaluations rely on synthetic benchmarks that fail to reflect the complexities of clinical practice. In a study last year, they found that a mere 5% of LLM analysis relies on actual world patient information, which reveals an enormous difference between testing real-world usability and indicates a profound problem with ascertaining how reliably they function in medical decision-making, therefore also questioning safety and effectiveness for use in actual-world clinical settings.State-of-the-art evaluation methods mostly score language models with synthetic datasets, structured knowledge exams, and formal medical exams. Although these examinations test theoretical knowledge, they dont reflect real patient scenarios with complex interactions. Most tests produce single metric results, without attention to critical details such as correctness of facts, clinical applicability, and likelihood of response bias. Furthermore, widely used public datasets are homogenous, compromising the generalization across different medical specialties and populations of patients. Another major setback is that most models trained against these benchmarks exhibit overfitting to test paradigms and therefore lose much of their performance in dynamic healthcare environments. A lack of whole-system frameworks embracing real-world patient interactions erodes confidence even further in employing them for practical medical use.Researchers developed MedHELM, a thorough evaluation framework designed to test LLMs against real medical tasks, multi-metric assessment, and expert-revised benchmarks to address these gaps. It builds upon Stanfords Holistic Evaluation of Language Models (HELM) and incorporates a systematic evaluation across five primary areas:Clinical Decision SupportClinical Note GenerationPatient Communication and EducationMedical Research AssistanceAdministration and WorkflowA total of 22 subcategories and 121 specific medical tasks ensure broad coverage of critical healthcare applications. In comparison with earlier standards, MedHELM employs actual clinical data, assesses models both by structured and open-ended tasks, and applies multi-aspect scoring paradigms. The holistic coverage makes it better capable of not only measuring the recall of knowledge but also of clinical applicability, reasoning precision, and general everyday practical utility.An extensive dataset infrastructure underpins the benchmarking process, comprising a total of 31 datasets. This collection includes 11 newly developed medical datasets alongside 20 that have been obtained from pre-existing clinical records. The datasets encompass various medical domains, thereby guaranteeing that assessments accurately represent real-world healthcare challenges rather than contrived testing scenarios.The conversion of data sets into standardized references is a systematic process, which involves:Context Definition: The specific data segment the model must analyze (e.g., clinical notes).Prompting Strategy: A predefined instruction directing model behavior (e.g., Determine the patients HAS-BLED score).Reference Response: A clinically validated output for comparison (e.g., classification labels, numerical values, or text-based diagnoses).Scoring Metrics: A combination of exact match, classification accuracy, BLEU, ROUGE, and BERTScore for text similarity evaluations.One example of this approach is in MedCalc-Bench, which tests how well a model can execute clinically significant numerical computations. Every data input contains a patients clinical history, a diagnostic question, and a solution verified by an expert, thus enabling a rigorous test of medical reasoning and precision.Assessments conducted on six LLMs of varying sizes revealed distinct strengths and weaknesses based on task complexity. Large models like GPT-4o and Gemini 1.5 Pro performed well in medical reasoning and computational tasks and showed enhanced accuracy in tasks like clinical risk estimation and bias identification. Mid-size models like Llama-3.3-70B-instruct performed competitively in predictive healthcare tasks like hospital readmission risk prediction. Small models like Phi-3.5-mini-instruct and Qwen-2.5-7B-instruct fared poorly in domain-intensive knowledge tests, especially in mental health counseling and advanced medical diagnosis.Aside from accuracy, response adherence to structured questions was also varied. Some models would not answer medically sensitive questions or would not answer in the desired format, at the expense of their overall performance. The test also discovered shortcomings in current automated metrics as conventional NLP scoring mechanisms tended to ignore real clinical accuracy. In the majority of benchmarks, the performance disparity between models remained negligible when employing BERTScore-F1 as the metric, which indicates that current automated evaluation procedures might not fully capture clinical usability. The results emphasize the necessity of stricter evaluation procedures incorporating fact-based scoring and unambiguous clinician feedback to ensure more reliability in evaluation.With the advent of a clinically guided, multi-metric assessment framework, MedHELM offers a holistic and trustworthy method of assessing language models in the healthcare domain. Its methodology guarantees that LLMs will be assessed on actual clinical tasks, organized reasoning tests, and varied datasets, instead of artificial tests or truncated benchmarks. Its main contributions are:A structured taxonomy of 121 real-world medical tasks, improving the scope of AI evaluation in clinical settings.The use of real patient data to enhance model assessments beyond theoretical knowledge testing.Rigorous evaluation of six state-of-the-art LLMs, identifying strengths and areas requiring improvement.A call for improved evaluation methodologies, emphasizing fact-based scoring, steerability adjustments, and direct clinician validation.Subsequent research efforts will concentrate on the improvement of MedHELM by introducing more specialized datasets, streamlining evaluation processes, and implementing direct feedback from healthcare professionals.     It began with a song that didnt come out last year. It began with a song that didnt come out in this century. But what a song to open the 97th annual Academy Awards: Ariana Grande threading a wilting rendition of Somewhere Over the Rainbow. The moment obviously was a nod to the other Oz-centric movie on peoples minds these daysand one that Grande was nominated forWicked. And to be sure, theres powerful, internet-friendly magic in both Rainbow and the Wicked song that Cynthia Erivo soon joined Grande on stage to belt, Defying Gravity. Yet this didnt feel only like a play at ratings. It felt like a promise that much of the following four hours lived up to. Were here, the telecast seemed to insist, to celebrate the joy of going to the movies.How interesting it is, then, that Hollywoods biggest night would afterward be dominated by two indie studios most intent on living up to that ideal, including by a film championing a sex worker from Brighton Beach who will not be denied.Anora was indeed the biggest winner of the evening, dominating in every category within which Sean Baker was nominated: Best Original Screenplay, Best Editing, Best Director, and of course Best Picture (Baker is also a producer on the film). It was a grand slam for one of the biggest defenders of independent and theatrical cinema, with his leading lady and the woman responsible for bringing the formidable Ani Mikheeva to life, Mikey Madison, also taking home the Best Actress Oscar in what appeared to be a nailbiter race against The Substances Demi Moore.It was a sweep that might have surprised even some of the Anoras biggest boosters from a few months agowhich includes the critic youre currently readinggiven the films rather downbeat and somewhat cynical disposition about a capitalist system that puts Cinderella-like dreams in the eyes of Madisons tenacious protagonist, but leaves her with a bitterly cold reality come morning.But then again, Anoras biggest competition makes Bakers slow-boiling screwball comedy look like a fairytale in comparison. The other major winner of the night was Brady Corbets The Brutalist, a massive three and a half hour epic that deconstructs the American dream and the predatory nature of mixing commerce with art. While Corbet was shut out of Picture and Director, his grandiose vision still picked up a Best Actor Oscar for Adrien Brody (his second little gold man from the Academy), as well as prizes for Best Cinematography, and Best Original Score.In many ways, this falls into place with Academy trends over the last decade, which has seen one other Neon acquisition win Best PictureBong Joon-hos Korean film, Parasiteand two A24 releases, Moonlight and Everything Everywhere All at Once. Yet it seemed to speak perhaps to this moment in the industry writ-large.As aforementioned, tonight felt like a sincere celebration for the love of the movies. Grande opened things by reprising one of the greatest songs ever written for a motion picture, courtesy of Harold Arlen and Yip Harburgs Hollywood songbook in The Wizard of Oz. But it continued through maestro Quincy Jones getting his own prime spot in the telecast where Oprah Winfrey and Whoopi Goldberg eulogized the composer and then introduced a propulsive rendition of Ease on Down the Road from The Wiz. (This 90s kid also appreciated the Academy orchestra playing Jones iconic Austin Powers suite earlier in the evening.)But perhaps most presciently was the Oscar telecast simply stopped for nearly 10 minutes as the Academy gave a rousing tribute to the James Bond franchise, complete with musical performances of Live and Let Die and Skyfall. There was even a mini-ballet led by The Substances Margaret Qualley.For the record, it is not a major birthday in the James Bond franchises history, nor was it in honor of any particular artist or filmmaker. It was, however, a tribute and perhaps a memorial service for the Broccoli familys legacy. After shepherding the Bond movies for more than six decades, the Broccolis (led now by Barbara and her stepbrother Michael G. Wilson) have reluctantly surrendered the rights for 007 to a reportedly aggressive Amazon MGM, which has big dreams of a shared universe orbiting around Bond.So there will be more Bond to come, of course. So much more. But as with Star Wars or Lord of the Rings before it, the content to come via streaming shows and spinoff movies will likely never recapture that classic cinema magic which the Broccoli family jealously guarded for the better part of a century. And the Academy chose to celebrate (and mourn?) this turning of the tide.In all respects, this telecast was a rousing salute to the movies, right down to a charming host in Conan OBrien and the sharp idea of having collaborators like, say, Lily-Rose Depp come out to praise Nosferatu costume designer Linda Muir. In past years, older Oscar winners would appear to praise colleagues who might literally be strangers to them. It was as if to say welcome to the club. Tonight, by contrast, was intelligently about people worked closest to the nominees praising their talent, as well as perhaps explain to the folks at home why a cinematographer or production designer is so important.Join our mailing listGet the best of Den of Geek delivered right to your inbox!Yet so much of that celebration was in honor of Hollywood legacies of the past: Oz, Jones, and 007. But in an Oscars telecast that two short months ago looked briefly like it could be dominated at last by Netflix before Emilia Prezs campaign implosion, it was the indies taking home the lions share of major awards above and below the line. Prez still saw Zoe Saldaa win Best Supporting Actress, as well as take home a Best Original Song Oscar. However, after earning 13 nominations, walking away with only two Oscars does seem like a mild disappointment. Meanwhile the only other times streaming services were mentioned were via those implicit eulogies or for gags, such as when host OBrien led a comedy segment where he took a bunch of Gen-Zers to Cinema-Streams, a place where you go to watch streaming from a phone that the the building holds.Meanwhile newly minted four-time Oscar winner Baker wasnt joking during one of his quartet of Oscar speeches. After thanking presenter Quentin Tarantino for introducing him to Mikey Madison by casting her in Once Upon a Time in Hollywood, Baker said with passion and steel: Watching a film in a theater with an audience is an experience. We can laugh together, cry together, scream and fight together, perhaps sit in devastated silence together. And in a time in which the world can feel very divided, this is more important than ever. Its a communal experience you simply dont get at home, and right now the theatergoing experience is under threat, movie theaters, especially independently owned movie theaters, are struggling and it is up to us to support them if we dont reverse this trend we will be losing a vital part of our culture. This is my battle cry: filmmakers keep making films for the big screen. I know I will. Distributors please focus first and foremost on the theatrical releases of your films. Neon did that for me, and I thank you from the bottom of my heart.It would seem many in the industry feel similarly. A Netflix release like Emilia Prez can tie for the second most amount of Oscar nominations ever. But when its time to go home for the night, the Academy is still leaving with the films and studios that are supporting original theatrical experiences. And if you havent noticed theyre looking a lot more indie these days.

  • Vo1d Botnet's Peak Surpasses 1.59M Infected Android TVs, Spanning 226 Countries

    Mar 03, 2025Ravie LakshmananMobile Security / BotnetBrazil, South Africa, Indonesia, Argentina, and Thailand have become the targets of a campaign that has infected Android TV devices with a botnet malware dubbed Vo1d.The improved variant of Vo1d has been found to encompass 800,000 daily active IP addresses, with the botnet scaling a peak of 1,590,299 on January 19, 2025, spanning 226 countries. As of February 25, 2025, India has experienced a notable surge in infection rate, increasing from less than 1% (3,901) to 18.17% (217,771). "Vo1d has evolved to enhance its stealth, resilience, and anti-detection capabilities," QiAnXin XLab said. "RSA encryption secures network communication, preventing [command-and-control] takeover even if [the Domain Generation Algorithm] domains are registered by researchers. Each payload uses a unique Downloader, with XXTEA encryption and RSA-protected keys, making analysis harder."The malware was first documented by Doctor Web in September 2024 as affecting Android-based TV boxes by means of a backdoor that's capable of downloading additional executables based on instructions issued by the command-and-control (C2) server.It's not exactly clear how the compromises take place, although it's suspected to either involve some kind of a supply chain attack or the use of unofficial firmware versions with built-in root access.Google told The Hacker News at the time that the infected "off-brand" TV models were not Play Protect-certified Android devices and that they likely used source code from the Android Open Source Project (AOSP) code repository.The latest iteration of the malware campaign shows that it's operating at a massive scale with an intent to facilitate the creation of a proxy network and activities like advertisement click fraud.XLab theorized that the rapid fluctuation in the botnet activity is likely due to its infrastructure being leased in specific regions to other criminal actors as part of what it said is a "rental-return" cycle where the bots are leased for a set time period to enable illegal operations, after which they join the larger Vo1d network.An analysis of the newer version of the ELF malware (s63) has found that it's designed to download, decrypt, and execute a second-stage payload that's responsible for establishing communications with a C2 server.The decrypted compressed package (ts01) contains four files: install.sh, cv, vo1d, and x.apk. It starts with the shell script launching the cv component, which, in turn, launches both vo1d and the Android app after installation.The vo1d module's primary function is to decrypt and load an embedded payload, a backdoor that's capable of establishing communication with a C2 server and downloading and executing a native library."Its core functionality remains unchanged," XLab said. "However, it has undergone significant updates to its network communication mechanisms, notably introducing a Redirector C2. The Redirector C2 serves to provide the bot with the real C2 server address, leveraging a hardcoded Redirector C2 and a large pool of domains generated by a DGA to construct an expansive network architecture."For its part, the malicious Android app carries the package name "com.google.android.gms.stable" in what's a clear attempt to masquerade as the legitimate Google Play Services ("com.google.android.gms") to fly under the radar. It sets up persistence on the host by listening for the "BOOT_COMPLETED" event so that it automatically runs after each reboot.It's also engineered to launch two other components that have a similar functionality as that of the vo1d module. The attack chain paves the way for the the deployment of a modular Android malware named Mzmess that incorporates for four different plugins -Popa ("com.app.mz.popan") and Jaguar ("com.app.mz.jaguarn") for proxy servicesLxhwdg ("com.app.mz.lxhwdgn"), whose purpose remains unknown due to its C2 server being offlineSpirit ("com.app.mz.spiritn") for ad promotion and traffic inflationThe lack of infrastructural overlaps between Mzmess and Vo1d has raised the possibility that the threat behind the malicious activity may be renting the service to other groups."Currently, Vo1d is used for profit, but its full control over devices allows attackers to pivot to large-scale cyber attacks or other criminal activities [such as distributed denial-of-service (DDoS) attacks]," XLab said.     The Academy Awards were about to announce the winner of the coveted Best Actressprize when suddenly, the shows live feed on Hulu went out for many subscribers (including this writer) without warning or explanation.Users suddenly saw a blue screen that read Thank you for watching! This live event has now ended. You may exit playback and select something else to watch. But of course, the show hadnot ended. There were still several major awards to go, including the top prize for Best Picture of the year.If you (or me, I did this) tried to reload the feed, it sent you to an old Oscar preview show. If you went back to the Hulu.com where the Oscars telecast had previously occupied the top of the page there was no longer any mention of the Academy Awards. If you searched for Oscars on Hulu, it took you to a trailer for the show.This issue was widespread enough that social media was flooded with subscribers complaining and sharing their own images of their busted feeds.READ MORE: All of the Winners at This Years Academy AwardsHulu has yet to comment on the reason for the issue, but The Verge noted that Hulu had previously listed the live stream as running from 7PM ET to10:32PM, which is when the feed suddenly ended for many users. (The Disney owned subscription service had over 53 million subscribers at the end of 2024.)Said many users were understandably upset. I just cancelled and got a refund for last month. That issue was supposedly resolved, per Deadline, around 9PM ET roughly two hoursafter the man show had begun.And then, about 90 minutes later, the whole show cut off from many Hulu customers.(Anoras Mikey Madison won the Best Actress prize, by the way. And ironically, only a few minutes before that,Anoradirector Sean Baker had used one of his acceptance speeches to remind people that movie theaters are still the best place to see a film.
